Hi,

My name is Dionetta Mallory, L.C.S.W., and if you are reading this letter it is because you have chosen me as your provider for mental health services. I am a Licensed Clinical Social Worker (L.C.S.W.) in the state of California. I would like to welcome you to my program and commend you for seeking therapy services.

I have created this brief introduction to address common questions as you begin the process of receiving therapy services. This information will assist you in smoothly transitioning into receiving the care you seek. My goal is to reduce your anxieties as you make this transition. After all, I want to help you reduce your anxiety, not create more

  • Mon – Fri: 5:30 pm – 7:30 pm

  • Weekend: Hours vary

I provide Tele-health therapy via the online platform Simple Practice™. A link for the tele-health video session will be sent to you through Simple Practice™ after you make your appointment.

Cancellations

Please provide a 24-hr notice for any cancellations. If an appointment is cancelled closer to the appointment date, it will be considered a missed appointment. Late fees will be assessed after 3 missed appointments.

What is Simple Practice™?

Simple Practice™ is trusted online platform that many medical practitioners use to manage the business side of our practice. This type of online platform is also known as an Electronic Health Record (EHR) system. On Simple Practice™, you’ll be able to schedule appointments, complete forms, and submit payments with ease. Using Simple Practice™ allows me to focus on providing therapy services to my patients more personally and effectively.

Please be patient with me as I serve a wide variety of patients. If I miss your call, you are welcome and encouraged to leave a message and I will try to accommodate your request to the best of my ability.
